Instructions: The trainee must perform, simulate, or observe (as required and indicated by the X's) all practical factors listed on
this card. Activities indicating a single NPPO action may not be changed: activities indicating more than one NPPO action
(perform, simulate, observe) may be satisfied by completing any one. The order of preference for the actions shall be (1) perform,
(2) simulate, (3) observe.
Upon satisfactory completion of each activity, a qualified operator shall circle the action taken, and sign (using full signature) and
date in the appropriate space.
