operations is the historical performance of the activity. Emphasis should be placed on the

identification of repetitive or chronic performance problems, events, equipment failures, or

potential or previous programmatic deficiencies. Documents typically containing historical

performance information include the following:

Occurrence reports

Performance indicators and trends

Self-assessment reports

External assessment reports
