Use a case-study approach, performed by the facility safety coordinator
or another individual knowledgeable of industrial safety practices, which
addresses a personnel accident and includes a diagnosis of root causes
and a determination of activities to be accomplished by the STA. The
case study should include a discussion of the bases for the company's
safety policy, responsibilities of the STA following a personnel accident
(including immediate and follow-up actions), company policy and the role
of the STA in correcting accident causes, and the reasons for the
implementation of procedural requirements during and following an
accident.
