A structured review is performed to identify the root cause and corrective actions
to prevent recurrence of each abnormal event.
6.
A timely and comprehensive investigative report is prepared and disseminated,
including entry into the Occurrence Reporting and Processing System (ORPS).
7.
Events are evaluated to determine what training is appropriate.
8.
Follow-up review is performed to evaluate the effectiveness of corrective actions
and determine patterns of deficiencies.
9.
Acts of known or suspected sabotage are immediately investigated.
