Describe the performance problem.

How many employees are experiencing the performance problem? How many

employees are employed in the job position?

Did the employee(s) receive formal training? Was a systematic approach to training

used to build and maintain the training program? How long ago did the employee

receive the training? Has the employee been involved in any continuing training?

Estimate how much it costs because of this performance problem.

What attitudes are prevalent in your environment about the employees' performance?

Does management support the conduct of this analysis? How does the employee

feel about his/her performance?
