Facility Representatives at the nuclear fuel

facility recognized the potential for a high

airborne radioactivity condition being caused

by leak-by of a contaminated system

pressure relief valve. Air samples conducted

as a result of the Facility Representatives'

questions confirmed a high airborne

condition.

A Facility Representative observed a worker

who was working approximately 21 feet

above a concrete slab without using fall

protection equipment. The worker

immediately began using the safety

equipment and the safety manager briefed

workers on the incident.
