FINDING:
A worker was observed to work near an energized circuit of 110 volts and greater than 1 milliamp
without using safety gloves. (References: 1, 3)
DISCUSSION: During the operational retest of valves MOV-3 and 5, a worker removed
the cage access panel behind the control panel and entered the cage area which contains
unguarded and uninsulated energized parts. The worker then attached an ammeter clamp on
an energized circuit. When the two MOVs were operated the worker observed the current
through the ammeter. When the test was complete the worker removed the clamp and
exited the cage. The worker was not wearing rubber gloves as required by reference 3.
