APPENDIX 2: SITE CONTACT MEMORANDUM

Refer to the Plan of Action.

Operations Office, Site Office Manager, or Facility Manager.

Operational Readiness Review of [Facility]

An Operational Readiness Review (ORR) of [Facility] will be performed. [Site points-of-contact] have

been identified as the lead Department of Energy and contractor facility contacts at the site. [Team

Leader] will lead the review team.

Enclosed are a preliminary schedule for the overall review and a detailed schedule for the onsite portion,

an agenda for the pre-review visit, a list of support required for the onsite portion of the review, a list of

site documents required to prepare the team for the review, and an ORR summary document. Please

forward one copy of each requested document to [Team Leader] by [Date].
